Chronic Disease Management Plans
Radius Medical Centre offers chronic disease management plans to our patients, if your doctor has determined that you qualify under the Medicare criteria for a chronic disease management plan you will be entitled to claim for some of your allied health visits. For example, Podiatrist, Dietitian, Physiotherapist. Please be aware initiating this plan requires more than 1 appointment and you are required to return to your doctor for a 2nd appointment to finalise the process. The plan must be finalized before you attend your first Allied Health appointment. If you attend your Allied Health appointment before your doctor has signed off on your Care Plan with you, then unfortunately you will not be able to claim your appointment through Medicare.
